Payment Information
At Renew and Live, LLC, our Diabetes and Nutrition Education Program accepts United Health Care, BCBS, Aetna, and Amerigroup insurances. Our services are eligible for payment through your Flexible Spending Account (FSA) or Health Savings Account (HSA). We will provide you with a detailed billing statement and receipt for your visit, which you can submit to your insurance company or FSA/HSA provider to assist with the billing process.
In addition, we accept all major credit cards and provide the option to pay via Paypal or Zelle. Payment is required at the time of service.
To schedule an appointment or inquire about fees, please contact our office at 678-837-4024. We kindly ask that you give us 24 hours' notice if you need to cancel your appointment to avoid incurring additional charges.
